During her romantic dilemma, her estranged father, Thatcher, passes away, though they are able to make peace before his death.

Si scopre che l'ordigno innescato e inesploso si trova ancora dentro al corpo del paziente e viene attivato il codice nero in tutto l'ospedale.
Il paramedico, Anna, ha una crisi di nervi e dopo alcune ore decide di togliere la mano e scappare via.
Meredith, che in quel momento si trovava a fianco a lei, mette la mano al posto suo. Nelle ore successive Meredith riesce ad estrarre l'ordigno e a passarlo al capo degli artificieri, che lo trasporta ai colleghi che lo aspettano alla fine del corridoio.
Mentre l'uomo si dirige verso di loro, la bomba esplode, uccidendo sia lui che gli altri artificieri. Fortunatamente non ci sono altre vittime; la Bailey nel frattempo ha dato alla luce suo figlio, grazie all'aiuto di George, e Derek ha operato con successo suo marito, Tucker, rimasto coinvolto in un incidente mentre veniva in ospedale per assistere al parto della moglie.

Rimangono soltanto Meredith, Derek e Finn. Entrambi si offrono per accompagnarla a casa, e lei rimane ferma a guardarli, incapace di prendere una decisione.
